What is a form destination?

A form destination is a tap-or-scan endpoint that opens a data-collection form instead of a webpage, video, or file — it turns any NFC tap or QR scan into an instant lead-capture moment, no app download or typing a URL required.

A 2025 Formstack study of 1,500 B2B decision makers found average form abandonment reaches 67.8% once a form asks for more than seven fields, and separately, HubSpot research found each additional form field decreases conversion by an average of 4.1%.

How to create a form destination

Build

Create a native form in TacTile — fastest to set up and edit later.Recommended

  1. Open the TacTile app and tap Create.
  2. Select the Form category, then choose Build a form.
  3. Name your form (e.g., "Trade Show Lead Capture") — this label is internal only.
  4. Add fields one at a time: tap Add Field and choose a type — text, email, phone, dropdown, or checkbox.
  5. Reorder fields by dragging, and mark required fields with the toggle.
  6. Set a confirmation message or redirect shown after submission.
  7. Tap Save, then Publish to attach the form to your card, tag, or QR code.
Connect

Link a live Google Form so responses keep flowing into your spreadsheet.

  1. In TacTile, tap Create → Form → Connect Google Form.
  2. Sign in and authorize TacTile to read your Google account's form list.
  3. Select the specific Google Form you want to connect.
  4. Confirm the connection — TacTile will always pull the live version of that form.
  5. Tap Publish. Any edits you make later in Google Forms update automatically without republishing.

Which path should I use?

Build if you want the fastest, most native-feeling experience and don't already use a form tool — it's the quickest to set up and easiest to edit later from the same app. Connect if your team already lives in Google Forms and wants responses flowing into a spreadsheet you already manage. Link if you've invested in Typeform, JotForm, or another builder with logic or branching TacTile doesn't natively replicate, and you just want the tap to route there.

Common use cases

Trade show booths

Staff tap an NFC badge or visitors scan a table-top QR code to submit contact info in one motion, no exchanging business cards or typing on a tiny keyboard.

Real estate open houses

A yard sign or door placard lets visitors tap to register interest and request the listing sheet without an agent standing over their shoulder.

Restaurant email/waitlist signup

A table tent or host-stand tag lets guests join a waitlist or opt into a mailing list while they wait to be seated.

Event registration

A poster or lanyard tap sends attendees straight to a short RSVP or check-in form instead of a printed sign-up sheet.

Service business intake and quote requests

A van decal or invoice insert lets a homeowner request a quote the moment they think of it, rather than remembering to call later.

Form + analytics

Every tap or scan on a form destination reports back to your TacTile dashboard: total taps/scans, unique visitors, time of day, and device type. The form-specific metric to watch is submission rate — the percentage of people who opened the form and actually completed it, which lets you see directly whether trimming fields improves completion.

Frequently asked questions

Do people need the TacTile app to fill out my form?
No. Tapping or scanning opens the form in their phone's browser — no app download is required for the visitor.
Can I require an email or phone number before someone can submit?
Yes, in the Build path you can mark any field as required, including email and phone.
What happens if I connect a Google Form and then edit it later?
Changes sync automatically — TacTile always displays the current live version of the connected form.
Can I use a form I already built in Typeform or JotForm?
Yes, use the Link path and paste the form's public URL.
How many fields should my form have?
As few as possible. Given that abandonment jumps to 67.8% past seven fields and each extra field costs roughly 4.1% in conversion, most tap-to-fill forms should stay under five fields.
Will I get notified when someone submits?
Yes, notification settings are available per destination so you can get real-time alerts for new leads.
Can one card have both a form and other destinations?
Yes, a form can be one destination among several on the same digital card, or it can be the sole destination for a single-purpose tag.
